Clinical Pharmacist Care Homes & Frailty

Droitwich, Ombersley & theRurals (DO&R) Primary Care Network has an exciting opportunity for anexperienced Clinical Pharmacist to join our established multidisciplinaryfrailty team. This permanent, part-time role (24 - 28 hours per week) focuses ondelivering expert medicines optimisation and clinical pharmacy support forolder adults living with frailty across care homes and the wider PCN.

Working as a core member of thePCN frailty MDT, you will provide structured medication reviews, polypharmacymanagement and deprescribing support, working closely with GPs, AdvancedPractitioners, nurses, social prescribers, community services and care homestaff. The role supports proactive, personalised care to improve patientoutcomes, reduce medicines-related harm and prevent avoidable hospitaladmissions.

You will contribute to currentPCN contracts and frailty pathways, participate in MDT meetings and caseconferences, and support service development, quality improvement andprescribing safety initiatives. This role offers the opportunity to applyadvanced clinical pharmacy skills within a supportive PCN environment, withaccess to supervision, appraisal, continued professional development and theNHS Pension Scheme.

Salary range -£ 46,226to£53,846 dependent on qualifications and experience

Main duties of the job

The post holder will provideclinical pharmacy and medicines optimisation support for older adults livingwith frailty across care homes and the wider Primary Care Network. Working aspart of the multidisciplinary frailty team, the role includes undertakingcomprehensive and structured medication reviews, supporting polypharmacymanagement and deprescribing, and working closely with GPs, nurses, socialprescribers, community services and care home staff. The post holder willcontribute to MDT working, personalised care planning, and prescribing safety,with a focus on improving outcomes, reducing medicines-related harm andpreventing avoidable hospital admissions.

Job Summary

The post holder will provide clinical pharmacy and medicines optimisation support for older adults living with frailty across care homes and the wider Primary Care Network. Working as a core member of the multidisciplinary frailty team, the role supports proactive, personalised care to improve outcomes, reduce medicines-related harm and prevent avoidable hospital admissions.

Key Responsibilities

  • Provide expert advice and support on prescribing and medicines-related issues for patients with moderate to severe frailty.
  • Undertake comprehensive and structured medication reviews, including polypharmacy management and deprescribing where clinically appropriate.
  • Work collaboratively with GPs, Advanced Practitioners, nurses, social prescribers, link workers, community services and care homes.
  • Participate in multidisciplinary team meetings, case conferences and proactive frailty reviews.
  • Support personalised care planning, anticipatory care planning and shared decision-making with patients and carers.

Clinical and Professional Responsibilities

  • Apply frailty-specific prescribing principles in line with NICE guidance, STOPP/START criteria and anticholinergic burden reduction.
  • Contribute to service development, quality improvement activity and prescribing safety initiatives.
  • Support delivery of PCN contract requirements and work within clinical governance frameworks and PCN policies.
  • Maintain professional registration, revalidation requirements and continued professional development.
Person Specification
Qualifications
  • Qualifications ESSENTIAL
  • Registered Pharmacist with the General Pharmaceutical Council (GPhC)
  • Completion of an undergraduate degree in Pharmacy
  • Completed CPPE PCPE pathway or equivalent
Experience
  • Experience in primary care, community or hospital pharmacy
  • Experience of undertaking medication reviews and polypharmacy management
  • Strong understanding of frailty, long-term conditions and older adult care
  • Experience of working within a multidisciplinary team
  • Experience working with frail older adults or care homes
  • Knowledge of social prescribing and personalised care models
  • Experience contributing to service development or quality improvement
  • Experience using GP clinical systems (e.g. EMIS, SystmOne)
  • Member of the Royal Pharmaceutical Society (RPS)
  • Working towards or holding a postgraduate pharmacy qualification
Communication and deadlines
  • Excellent communication skills and ability to work collaboratively within a multidisciplinary team
  • Ability to work within professional limits, clinical governance frameworks and organisational policies
