Job description

A busy and well-established primary care practice in MK40 is seeking an experienced Clinical Pharmacist to join its multi-disciplinary team on a permanent basis. This role will support safe prescribing, medicines optimisation and direct patient care.

Key Responsibilities
  • Conduct clinical medication reviews (face-to-face, phone or notes-based)
  • Provide specialist medicines advice to clinicians and patients
  • Lead medicines optimisation and safety initiatives
  • Review discharge/specialist treatment letters and action medication changes
  • Support public health activities (e.g. flu and routine immunisations)
  • Run medicines-focused clinics (e.g. anticoagulant monitoring)
  • Support dispensary team when needed
  • Triage minor ailments and support long-term condition care
