A local healthcare provider in Woking is seeking a Chronic Disease Practice Nurse to manage care for patients with long-term conditions. Responsibilities include developing care programs, assessing patient needs, and providing routine nursing care. Successful candidates must have a Registered Nurse qualification and experience in chronic disease management. This role offers an opportunity to impact patient care positively within a general medical practice.
To be responsible for the implementation ofprocesses for the effective management of patients with long-term conditionsusing evidence-based practice including care for elderly and houseboundpatients.
Working as a senior member of the practicemultidisciplinary team, the post holder will ensure nursing services aredelivered effectively to the entitled patient population.
The chronic disease practice nurse will beresponsible for a number of clinical areas as well as actively supporting themanagement team in reviewing and delivery of clinical policy and procedure.

Key Responsibilities:
To SupportInfection Prevention Control
To develop,implement and embed an effective care management programme for the frail andhousebound patients registered at the practice
To assess theneeds of patients ensuring the provision of healthcare is appropriate.
To develop,implement and embed health promotion and wellbeing
To implement andevaluate individual treatment plans for chronic disease patients
To identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to patients health
Provide routinenursing care to patients in accordance with clinical based evidence
As requireddiagnose and manage acute and chronic conditions
To prescribemedication for patients in accordance with local formularies, national guidanceand own scope of practice
To review patientmedications to enhance compliance
To provideguidance to patients in the use of prescribed and over the counter medicationsregarding side effects and dosages
To co-ordinatethe provision of travel medicine services
To requestpathology services and process pathology results as required
To maintainaccurate clinical records
To assist GPswith minor surgery when required
To support theteam in dealing with clinical emergencies
To implementvaccination programmes for adults and children
The Chobham & West End Medical Practice isa generalpractice with a main surgery in Chobham and a branch site in West End, both ofwhich are purpose-built facilities.The practice provides general medicalservices to the local community.
The practice is a part of the NHS (NationalHealth Service) and is regulated by theCare Quality Commission. The Chobham & West End MedicalPractice is a relatively modern practice, with its main surgery in Chobham andbranch site in West End, both being purpose-built facilities, as mentioned intheCare Quality Commissionreport.
The following are the coreresponsibilities of the Chronic disease practice nurse. There may be, onoccasion, a requirement to carry out other tasks; this will be dependent uponfactors such as workload and staffing levels.
