Overview

The Central Norfolk Home First Hub is recruiting. We are looking for experienced and reliable Occupational Therapist/Physiotherapist to join our Norwich Place therapy teams. The role provides support to the Central Norfolk Home First Hub, covering the Norwich, South and North places, and will involve working across a 7 day week. There are opportunities to maintain clinical competencies by supporting work in both Procured Beds and Urgent Community Response Therapy Team.

The service is operational during the following hours: Monday to Friday 08:00 – 18:00, Saturday 09:00 – 17:00, Sunday 10:00 – 16:00.

Key Areas of Responsibility
  • Triage and allocation of referrals for patients on Discharge to Assess pathway, assessing the patient’s needs, determining the priority of care, processing and allocating referrals accordingly, liaising with multidisciplinary team to ensure timely responses to referrals based on individual patient needs using the knowledge, skills and expertise of others in the Integrated team.
  • Assess and prescribe care packages for patients with long term conditions, the frail & elderly, palliative care and rehabilitation needs to achieve quality of life and independence where possible.
  • Work within the integrated team to facilitate early discharge from hospital.
  • Work within the integrated team to prevent unnecessary admission to hospital.
  • Assess and treat patients in their own homes/care homes.
  • Communicate with all health care professionals and statutory/non-statutory agencies to provide a seamless, integrated service to service users on the Discharge to Assess pathway and in community teams.

  • Ability to prioritise own workload, communicate effectively, work as part of a team, and use initiative.

The Central Norfolk Home First Hub is an integrated multidisciplinary team of health and social care colleagues. It consists of clinical leads, practice consultants, OTs, Physios, Community Assistant Practitioners and Social Care Assistant Practitioners, supported by Integrated Care Coordinators and business support. It is a newly established and evolving team. Successful applicants will receive regular supervision opportunities.
