Job description

We are seeking Band 5 nurses to join our district nursing teams based in Liverpool Place, covering our 8am-8pm, 7 days a week model. The service is undergoing significant development as part of our plan to deliver integrated community care across South Liverpool.

Working in line with Mersey Care values, you will demonstrate excellent empathy, communication skills, and a person-centred approach. A commitment to learning and developing the competencies necessary to provide evidence-based care in patients' homes is essential. Clinical decision-making and problem-solving skills are critical components of this role. You should be a good team player capable of working independently, escalating issues appropriately.

The service operates 24/7, 365 days a year, to support our extended hours model.

You will have opportunities for mentorship and full support to develop and enhance your skills.

The post holder is responsible for delivering clinical care to a defined population, working within their role's boundaries and supported by senior practitioners. Effective collaboration within the team is essential to meet patient needs, support policy and procedure implementation, and provide clinical leadership. The role also involves ensuring the delivery of quality strategies within the area, establishing systems and processes.

Applicants must hold a full driving license.
