Job description
Overview

Care Plan Coordinator – 30 hours per week. Working in a residential care home in Heeley, S2. Shift pattern: 10:00am to 3:00pm, 5 shifts per week (may include occasional evening or weekend work and bank holidays). Pay: £12.40 per hour.

All new team members are required to attend an induction at our training room in S13 prior to starting work in the care home. Inductions run every three weeks, starting on a Monday between 9:30am and 3:30pm.

Responsibilities
  • To arrange and facilitate residents assessments and admissions.
  • To take responsibility for updating Resident Monthly Care Plans.
  • To communicate with families for their input in care planning and send them regular updates.
  • To carry out assessments of potential residents.
  • To communicate with Team Leaders and Care Staff regarding changes in residents’ medication, health and well‑being.
  • To promote and uphold Sheffcare’s values.
  • To implement and uphold safe working practices in line with our regulations and our policies and procedures.
  • To attend regular training where required.
  • To support positive relationships with our residents, their family, colleagues and professionals.
  • To record all updates using mobile technology.
