A local charity organization in Sheffield seeks a Care Plan Coordinator to work 30 hours per week in a residential care home. The role involves facilitating assessments, updating care plans, and ensuring communication with families and staff about residents' needs. Ideally, candidates should have strong interpersonal and organisational skills. This position offers a permanent contract, extensive training, and community engagement.
Care Plan Coordinator – 30 hours per week. Working in a residential care home in Heeley, S2. Shift pattern: 10:00am to 3:00pm, 5 shifts per week (may include occasional evening or weekend work and bank holidays). Pay: £12.40 per hour.
All new team members are required to attend an induction at our training room in S13 prior to starting work in the care home. Inductions run every three weeks, starting on a Monday between 9:30am and 3:30pm.
