Job description

The Old School Surgery and UWE Health centre Frenchay, are looking to appoint a highly motivated Care Navigator to enhance and support our hardworking & friendly reception team.

Full and part time hours considered, and this will also include very occasional Saturdays.

Responsibilities
  • Be the first point of contact for our patients, whether it be face to face or by telephone, offering support and guidance on choosing the right service at the right time.
  • Deal with administrative tasks behind the scenes, processing incoming documents from third parties, prescriptions, GP Requests and more.
  • Remain friendly and welcoming to all patients, staying calm and professional when dealing with difficult situations.
  • Use our online patient access system (Klinik) to monitor online patient requests and triage patients accordingly.
  • Book appointments for GP clinics and other practice clinicians as required, ensuring correct information is provided.
  • Ensure accurate information and messages are passed to the Doctors and other surgery staff.
  • Deal with all telephone queries/requests from patients and act as a liaison with GPs and other practice clinicians.
  • Participate in in-house training or other training as required.
  • Respect and maintain all aspects of confidentiality relating to patients, staff and practice.
  • Perform any other duties reasonably expected of the post.

The Old School Surgery is based in a beautiful old Victorian school, set opposite the lovely central Fishponds Park and St Marys Church. We have a branch surgery at the University of the West of England housed in the middle of this vibrant University.

Responsibility for local care homes.

Strong links with local social prescribing service with social prescriber working within the practice.
