Job description

As a Care Coordinator you will play a key role in supporting patients toensure they recieve the right care and services tailored to theirindividual needs. You will provide professional and courteousadministrative and front-linesupport services for patients, visitors and the wider practice team,ensuringall duties are performed effectively and to a good standard meeting theobjectives of the practice.

Main duties of the job

The post-holder will be responsible for coordinating patient care, facilitating communicationbetween healthcare professionals, and ensuring patients and their families are well informed and supported.

This role will focus on maintainingsmooth and efficient care pathways. Once trained, the post-holder willalso provide guidance and support to colleagues on best practices in care coordination.

We employ a host of highly skilled staff includingpharmacists, pre-reg pharmacists, nurses, HCAs and other roles. We are atraining practice and support medical students and GP resident doctors. Job responsibilities
  • To help coordinate the patients journey through primary and secondary care.
  • Co-ordinate and prepare PPG meetings and agendas and attending meetings as a practice representative
  • Manage incoming correspondence coding and updating the record as appropriate
  • Manage paper notes coming in and leaving the surgery
  • Summarising incoming medical records
  • Process requests for private medical reports and subject access requests
  • Sending patient referrals and dealing with associated enquiries
  • Process registrations for new, temporary and immediately necessary patients
  • Deal with administrative queries and requests from patients
  • Conduct health and safety checks such as legionnaires testing, fire alarm tests and health and safety walk rounds
  • Ensure notice boards are accurate and up to date
  • Maintain and improve the practice website, including updating text, images, and ensuring technical functionality.
  • Create engaging and informative social media content
  • Create and promote our quarterly practice newsletter
  • Assist in the delivery of QOF, IIF and other enhanced services the surgery are to offer
  • Deal effectively with incoming patient calls and queries, seeking advice when necessary
  • Opening and closing procedures for the building ensuring doors, windows and alarms are secure
  • Ensure that all patients and visitors at reception and telephone callers to the practice are greeted professionally and politely
  • Signposting to relevant services or healthcare providers
  • Make/cancel/rearrange appointments via the clinical system
  • Arrange interpreters as necessary
  • Processing prescription requests
  • Deal with administrative queries and requests from patients
  • Support and assist clinicians and management as required
  • Taking delivery of goods according to practice policy
  • Ensure up to date patient information is recorded
  • Give test results where appropriate
  • Opening and closing procedures for the building ensuring doors, windows and alarms are secure

  • Providing cover for staff leave as necessary
