Job description
Overview

The service supports working-age adults who have severe affective and psychotic disorders. The teams offer support drawing from the expertise of a variety of disciplines.

Responsibilities
  • Deliver care-coordination to a specified caseload operating under the CPA framework, including a daily duty service, ongoing risk management, care planning, and support around benefits and housing.
  • Provide carers’ assessments and physical health care, and deliver a wide variety of therapeutic interventions with access to training and supervision.
  • Engage service users and their families using assertive outreach approaches to build a therapeutic and recovery-based relationship.
  • Contribute to the maintenance and development of the Trust nursing strategy and ensure clinical practice is evidence-based and aligned with NICE guidelines and Trust policies.
  • Supervise junior staff and trainees where appropriate and act within the professional code of conduct at all times.
  • Work in a supportive environment that values supervision and professional development, with a voice in service development.
  • Support the service in ensuring a positive learning environment for students from various disciplines.
Qualifications and requirements
  • Degree or relevant professional registration as a Nurse, Occupational Therapist or Social Worker.
  • Active membership of a dynamic, motivated, skilled and creative team.
  • Willingness to travel independently around the county to meet service needs and meet strict time deadlines.
  • Full UK driving licence and use of a vehicle (public transport is not an acceptable alternative for this role).
