Job summary

Job description

The service supports working-age adults with severe affective and psychotic disorders. Teams offer multidisciplinary support, drawing from various expertise.

• You have a degree or relevant professional registration as a Nurse, Occupational Therapist, or Social Worker and are an active team member.

• We support agile working with appropriate IT equipment and have secure facilities for staff bicycles. The city encourages active commuting, including cycling.
• You will deliver care coordination under the CPA framework, including risk management, care planning, support with benefits and housing, carers’ assessments, physical health care, and therapeutic interventions. Training and supervision are available.
• Travel independently around the county is required, with a full UK driving license and vehicle. Public transport is not suitable for this role.

• Ensuring evidence-based practice aligned with NICE guidelines and Trust policies.
• Supervising junior staff and trainees where appropriate.
