Job description

The service delivers high quality care to patients at in their own home with a primary purpose to prevent hospital attendance/ admission and facilitate early discharge from hospital. The service operates from 8am to 10pm, 7 days a week.

The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Teams, other health care professionals and statutory and voluntary agencies to deliver excellent standards of patient care.

The post holder will be required to:

Work on a rotational shift basis within the Herts Unplanned Care Team - service operating hours 8am 10pm 7 day a week.

Be the clinical leader for designated shifts on a rotational basis responsible for day-to-day management of the service.

Take designated responsibility for discrete areas of operational management, line management and clinical practice.

Prioritise and manage own workload with minimal supervision.

Main duties of the job
  • Provide operational management and clinical leadership for the Unplanned Care Team including triage of referrals and ensuring efficient and effective use of resources via daily staff management of time keeping, work allocation and rest breaks, respond to immediate sickness absence (requesting cover as per protocol as required).
  • As an Advanced Unplanned Care Practitioner act as role model supporting the team to achieve and maintain high standards of clinical care, based on evidence-based practice.
  • As a senior team member and as an Advanced Practitioner the need to respond to clinical incidents, accidents and complaints in accordance with CLCH policy.
  • Work in collaboration with and maintain a good working relationship within the other CLCH services such as adult community nursing, GP out of hours services, Accident and Emergency Departments, London Ambulance Service, acute hospital assessment units/ wards, General Practitioners and practice staff, nursing and care homes, Social Services and voluntary sector organisations so as to provide a comprehensive service to patients, clients and carers.
  • Undertake assessment and appraisal of designated members of the team.

Skills & Knowledge
  • Evidence of comprehensive range of clinical skills and expertise relevant to the role.
  • Competent in physical examination of: ENT, eye, gastrointestinal, respiratory, cardiovascular, neurological, skin, musculoskeletal systems and wound management.
  • Specialist knowledge in clinical reasoning, history taking, physical examination, diagnostic skills and prescribing/treatment.
  • Ability to make formal presentations to a wide range of people.
  • Knowledge of resource management.
  • Able to lead and motivate a team and implement change.
Key Attributes
  • Desirable: Hold a full, valid UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Experience
  • Extensive experience in Primary Care and / or Accident & Emergency.
  • Proven experience of utilising expert diagnostic and clinical skills to provide autonomous, unscheduled medical care and management of patients presenting with undifferentiated and undiagnosed conditions.
  • Experience of assessing and providing quality care to people with Long Term Conditions.
  • Track record of practice excellence.
  • Effectively working in collaboration, negotiating, and liaising across organisational boundaries.
  • Experience in monitoring clinical standards.
  • Proven ability to prioritise and manage own workload with minimal supervision.
  • Determining best ongoing care for patient.
  • Team / service/ staff management.

Education/Qualification
  • Registered Nurse with current NMC registration OR Paramedic with HCPC registration
  • Evidence of continuing professional education and training at Masters Level.
  • Further desirable qualifications in Primary Care/ Accident & Emergency/ acute clinical assessment.
  • Qualification in teaching or practice-based learning.
