What You Will Do

Deliver clinical pharmacy services on acute hospital wards, with a focus on medicine, surgery, and orthopaedics.

Conduct medicines reconciliation and intervene as needed to ensure patient safety.

Review prescriptions and patient histories, making clinical recommendations as appropriate.

Support timely and efficient discharges through use of electronic prescribing systems.

Collaborate with multidisciplinary teams to deliver safe, effective, and person-centred care.
