Job description

Join our Solihull Community Services Intermediate Care team, working across multiple sites in Solihull and Birmingham. We provide therapy for patients needing further assessment and rehabilitation after a hospital discharge, to determine the appropriate community pathway/discharge destination for our patients. Our main focus is supporting complex discharge planning and ensuring smooth transitions from hospital to community care.,

Responsibilities
  • To deliver a high standard of therapy provision within the Solihull Community and Intermediate Care Bedded Units across Pathway 1 and 2.
  • To support our patients in their recovery journey and minimise the risk of hospital readmissions.
  • To make the transition from acute care to the community as smooth as possible, ensuring timely discharges and efficient flow.
  • To liaise with various agencies to ensure safe, effective and efficient discharges.
  • To provide Physiotherapy services in the patient's own home or residence.
  • To contribute to seven-day working programmes across the organisation.
  • To be part of On-call respiratory Rota.
  • To be able to manage your caseload and work independently.
  • To supervise students on placements.
  • To appraise, supervise and support junior staff and therapy assistants.
  • To initiate/participate in audits, research and quality improvement programmes.
Ideal Candidate

Ideal for an experienced Band 5 seeking progression or a Band 6 looking to expand expertise in managing complex discharges. Driving and access to a vehicle with business insurance is essential.
