Job description

To work alongside service users, families, carers and multi‑disciplinary team members to co‑ordinate care as a key worker, supporting collaborative decision‑making about care and treatment.

To deliver a set of wellbeing‑focused psychologically‑informed interventions, aligned to cognitive‑behavioural principles, based on the best evidence available, that address problems often experienced by people with severe mental health problems.

About the Position

Burnley Community Mental Health Team are seeking to appoint a Band 5 Mental Health and Wellbeing Practitioner. This post provides the opportunity for a forward‑thinking and experienced individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care. With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships.

You must be a registered mental health and well being practitioner, with excellent leadership and clinical skills. You will also need to have a working knowledge of the care programme approach, risk assessment and management, and the ability to explore issues relating to the impact of health and social needs on mental health and well‑being.
