Job description
Overview

An exciting opportunity to join the team and become part of the Mid Essex Dementia and Frailty Pathway. This developmental role is a band 4 position requiring a care certificate and relevant maths and English within 12 months to progress onto the apprenticeship.

Key Responsibilities
  • Work as part of a multi‑disciplinary team supporting patients with SMI, dementia and frailty across Mid Essex.
  • Assess and manage both physical and mental health needs, including annual physical health checks for patients with severe mental illness.
  • Record vital signs, venepuncture for blood tests, and complete electronic physical health assessment forms.
  • Engage patients in health promotion, healthy eating, sport and exercise, and smoking cessation.
  • Collaborate with key partner organisations, clinicians, and other practitioners to deliver a systems approach to patient wellbeing.
  • Support the clinical manager with administrative tasks and maintain IT/computer skills.
Qualifications
  • Valid UK driving licence and access to a car for work purposes.
  • Care certificate.
  • Relevant maths and English within 12 months.
  • Literate in IT and computer skills.
  • Ability to drive and work Monday‑Friday 9‑5 across Mid Essex.
