Job description
Overview

Our Community Mental Health Recovery Service comprises secondary Mental Health Teams that provide assessments for evidence-based, time-limited treatment and psychological interventions for adults with severe and enduring mental health issues. The focus of our service is promoting recovery and independence, and developing self-management strategies to enable individuals to achieve their potential in the community. We are a multidisciplinary team that provides advice and support for GPs, works with individuals and their carers, and can refer or signpost to statutory, independent and voluntary sector agencies. We seek to work closely with individuals, their families (where consent has been given) and other organisations to coordinate and monitor the delivery of person-centred healthcare to a designated group of service users, using complex clinical decision making where appropriate.

To work as a member of the MDT in assessing and planning care and responding to changes in the health of people who use our services.

Subject to reasonable adjustments in respect of any disability, candidates must possess a valid driving licence and be able to drive and have unrestricted use of a car or motorcycle for work at all times, including community-based work.
