Job Summary
Clinical Triage Therapist, Neuro Rehabilitation Service
They are looking for an experienced OT, PT or SLT to support us in a Clinical Triage role for the Community Neuro Rehab Team. This Band 6 position is a full‑time, fixed‑term contract covering a 6‑month maternity leave, with flexible and remote working arrangements considered.
Advance your clinical reasoning, risk assessment and communication skills while participating in a new operational model for the service. You will identify appropriate pathways, support MDT prioritisation of patients, and manage clinical cases within your professional field.
Work closely with MDT and admin colleagues, and remain part of your wider professional group. Regular supervision and training opportunities are provided.

Responsibilities
Clinical Triage
- Lead the triage service, prioritising and processing referrals received daily.
- Hold a small clinical caseload, providing comprehensive assessment, treatment and management to individuals with neurological conditions.
- Monitor intervention effectiveness through goal setting, outcome measurement and practice evaluation.
- Work collaboratively with individuals, carers, MDT members and external agencies to ensure coordinated service delivery.
- Educate patients and families about the impact of neurological conditions and promote healthy lifestyle.
- Enable self‑management and empower patients during treatment and after discharge.
- Respect patients’ dignity, privacy, culture and wishes with a commitment to equal access to occupational therapy.
- Maintain links with acute sector colleagues, local authority services, voluntary organisations and primary health care services.
- Ensure equipment safety, cleanliness and infection control standards, reporting issues to the Clinical Manager.
Professional Practice
- Act as an autonomous practitioner within professional and HCPC standards, being legally accountable for your work.
- Maintain liaison and effective communication with senior colleagues for smooth daily operations.
- Pursue personal growth through research, journal clubs, projects and a professional portfolio.
- Attend professional group meetings, training courses and share knowledge with colleagues.
Information Management
- Keep accurate and contemporaneous records of assessment and treatment; enter data into the electronic record system.
- Provide timely statistical data.
Person Specification
Knowledge / Qualifications (Essential)
- Graduate Diploma or Degree in Occupational Therapy / Physiotherapy / Speech Language Therapy
- State registration with HCPC
Knowledge / Qualifications (Desirable)
- Evidence of attendance at relevant post‑registration courses
- Member of a relevant Specialist Interest Group
- Knowledge of current policies and publications on neurological conditions
Experience (Essential)
- Broad post‑registration experience at B5 level or above
- Experience with neurological and stroke rehabilitation specialties
- Experience working within a multi‑disciplinary team (MDT)
- Experience in clinical triage within a team
Experience (Desirable)
- Experience working in a community setting
- Participation in service development or project work
- Experience in a ward‑based rehabilitation setting
Skills (Essential)
- Client‑centred practice knowledge and application
- Assessment, treatment and rehabilitation skills for neurological and complex conditions
- Good IT skills
- Knowledge and use of relevant assessment tools and outcome measures
