A leading NHS Trust in the United Kingdom is seeking a dedicated Clinical Pharmacist to manage services for the Elderly Care & Frailty Clinical Service Line. You will lead in medicines optimisation, support patient safety projects, and mentor junior staff. The ideal candidate will have clinical pharmacy expertise and excellent leadership skills. We are seeking a dedicated, enthusiastic, patient-focused and motivated clinical pharmacist to join our high-performing pharmacy team to manage the service provision to the Elderly Care & Frailty Clinical Service Line.
The post holder will lead in all aspects of medicines optimisation in the Elderly Care & Frailty areas and will further develop our well-established role in the transfer of care around medication and the Discharge Medicines Service.
