We are seeking an experienced AdvancedPractitioner to join our busy team. You will work autonomously to assess, diagnose,and manage patients across a range of conditions, including acute, chronic, andurgent presentations. This role combines clinical expertise with leadership,education, and service development responsibilities.
Conduct comprehensive patientassessments and develop management plans.
Prescribe medicationsindependently (NMP qualification required).
Provide leadership andmentorship to clinical staff.
Contribute to qualityimprovement, audits, and service innovation.
Support education and trainingwithin the PCN.
Collaborate withmultidisciplinary teams and external agencies.

To provide advanced clinical care,leadership, and education within the primary care setting, ensuring safe,effective, and evidence-based practice.
Autonomous assessment,diagnosis, treatment, and referral.
Prescribing within scope ofpractice.
Manage acute and chronicconditions, urgent care, and preventive health.
Act as a clinical role model.
Support recruitment, induction,and appraisal processes.
Deliver training sessions andmentorship.
Promote continuous learning anddevelopment.
Lead audits and serviceevaluations.
Ensure compliance with clinicalgovernance standards.
