Advanced Nurse Practitioner - General Practice

The Panton Practice are looking torecruit a dynamic forward-thinking Advanced Nurse Practitioner to join theirexpanding multi-disciplinary team. We are looking for a person withexperience in dealing with acute minor illnesses. We operate on a system of e-Consultations, telephone triaging and same day face to face appointments.

Main duties of the job

The candidate would need to be ableto provide evidence of autonomous practice and be comfortable in patientassessment, use of diagnostic equipment such as ECG to facilitate this andinitiating treatment plans although full support will be accessible through theduty team on the day.

This role benefits from good communication skills, IT knowledge and the ability to multi-task, as well as be confident to work on their own, but also as part of a supportive and flexible team. Experience of SystmOne and working in the NHS would be an advantage but is not essential.

Primary Care is evolving, and we are part of the Bournemouth Central Primary Care Network, working with our other local practices to offer further extended services both in and outside of our core hours. These include Physio, Pharmacists, Social Prescribers, Health Coaches and our Frailty Team.

We are a teaching practice for GPs and allied professionals and are developing a strong Multi-Disciplinary Team and feel this is key for better patient care. Every day is different, and you come across new challenges which makes it a very diverse environment. We work in a friendly atmosphere where social events happen throughout the year.

Job responsibilities

Job Summary:

The primary focus of the role is to workas an autonomous practitioner providing high quality and timely patient centredcare.

The post holder will be responsible forthe assessment, diagnosis, treatment, referral and discharge of a diverse rangeof patients with minor injuries/minor illnesses, without reference to a doctorand within their level of competence and agreed protocols.

ClinicalCare:

Act as an Advanced Nurse Practitioner, demonstrating advanced clinical competence and a knowledge basebeyond those associated with conventional roles.

Undertake consultations with patients either by e-Consultation,telephone triage or face to face, as an autonomous practitioner and using ownclinical judgement to diagnose, treat, refer and/or discharge patients.

Make direct referrals to primary, secondary and social services withinlocally agreed pathways, guidance and protocols.

Provide clinical advice to Reception staff where required.

Ensure that patients receive high quality clinical care, delivered in atimely manner.

Supply and administer medicines as indicated to address patient need,where necessary working to Patient Group Directions.

Ensure complete and accurate documentation of each and every patientcontact.

Work as an autonomous practitioner and as part of multi-disciplinary andmultiagency teams in order to ensure patients needs are met.

Professional:

Adhere to the NMC Code of Professional Conduct and at all timeswork within the scope of professional practice.

Ensure that professional practice adheres to organisational and NMC policies, procedures and guidelines.

Maintain a professional manner and act as a positive role model forjunior staff and other members of the practice team.

Maintain confidentiality with regard to information pertaining topatients and staff.

Ensure that the service interfaces with all other departments in aprofessional and productive manner, providing an effective service to partnerorganisations and other service providers

Where required provide supervision and mentoring to other staff in orderto help them develop their knowledge, skills and abilities.

Communication:

Communicate effectively with patients and families regarding complexinformation about their care.

Provide advice and information to patients, carers and their familieswhere appropriate.

Communicate effectively and work collaboratively with medical, nursing, alliedhealth professionals and operational staff to ensure delivery of a co-ordinatedservice.

Ensure accurate record keeping in line with local policies and NMC guidelines.

Promote and demonstrate effective communication networks within the organisation,with partner organisations and other service providers.

Educational/PersonalDevelopment:

Adhere to professional registration requirements.

Participate in defining own developmental plan to maintain and developown clinical skills and ensure own professional development.

Undertake and complete all statutory and mandatory training.

Participate in an appraisal and regular performance reviews.

Ensure that all mandatory training is completed in line withorganisational policy.

Other

It is the responsibility of all staff to adhere to infection controlpolicies in order to promote cleanliness and reduce infections. Hand hygiene must be undertaken correctly toprevent the spread of infection. Staffmust contribute to the cleanliness of the work environment and keep it clutterfree and tidy and must also attend all mandatory training and updates toensure receipt of training appropriate to role.
