We are looking for an experienced Advanced Nurse Practitioner (ANP) to join our friendly and supportive general practice team. In this role, you will provide advanced clinical care, hold patient clinics, support chronic disease management, and contribute to quality improvement and public health initiatives. Main duties of the job

Assess, diagnose, and manage patients with acute and chronic conditions.

Hold patient clinics, including chronic disease reviews and medication management.

Prescribe medications and treatments within your competency and independent prescribing qualifications.

Provide clinical advice and support to the practice team.

Participate in clinical audits, quality improvement initiatives, and service development.

Contribute to public health and screening programmes, including immunisations and vaccinations.

Mentor and support junior staff and work collaboratively within the multidisciplinary team.

Qualifications
