Job summary
We are seeking an experiencedand motivated Advanced Clinical Practitioner (ACP) to join our innovative andfriendly general practice team. The successful candidate will deliverhigh-quality, patient-centered care and operate with a high degree of autonomyin a primary care setting.
The ACP will use specialistknowledge and clinical skills to manage a diverse caseload, providing care forpatients with undiagnosed, undifferentiated, and complex conditions. You willbe a member of the clinical team contributing to the development of ourclinical team and service delivery.
Main duties of the job

- Clinical practice:As an autonomous practitioner, you will manage a wide range of health needs through face-to-face appointments, telephone consultations, and home visits.
- Assess, diagnose, and treat patients presenting with undifferentiated and undiagnosed problems and follow up of patients after initial presentation.
- Undertake physical examinations and interpret diagnostic results to formulate and implement treatment plans.
- Ideally to be able to act as an independent prescriber, reviewing and amending medication as clinically appropriate, although this is not essential.
- Manage patients with complex and long-term conditions.
- Act as a clinical expert, supporting the delivery of high-quality, evidence-based care.
- Contribute on clinical policy and service development within the practice.

- Contribute to clinical governance by undertaking audits and quality improvement projects.
- Implement evidence-based practices to ensure the highest standards of care.

- Participate in the training and education of other healthcare professionals.

Job responsibilities
The following are the core responsibilitiesof the ACP in delivering health services.
- a. Practise in compliance with their respective code ofprofessional conduct and within their scope of practice, being responsibleand accountable for their decisions, actions and omissions at this level ofpractice
- b. Demonstrate a critical understanding of their broadenedlevel of responsibility and autonomy and the limits of own competence andprofessional scope of practice, including when working with complexity, risk,uncertainty and incomplete information
- c. Act on professional judgement about seeking help,demonstrating critical reflection on own practice, self-awareness,emotional intelligence, and openness to change
- d. Work in partnership with individuals, families and carers, using arange of assessment methods as appropriate

- n. Maintain accurate clinical records in conjunction withextant legislation ensuring that SNOMED CT codes are used effectively
- o. Chaperone patients where necessary
- p. Prioritise health issues and intervene appropriately
- q. Support the team in dealing with clinical emergencies
- r. Recognise, assess and refer patients presenting withmental health needs
- s. Support in the provision of vaccination programmes for bothadults and children
- t. Ideally be an extended and supplementary prescriber,adhering to extant guidance
- u. Support patients in the use of their prescribed medicinesor over the counter medicines (within own scope of practice), reviewingannually as required
- v. Liaise with external services/agencies to ensure thepatient is supported appropriately (vulnerable patients, etc.)
- w. Delegate clinical responsibilities appropriately(ensuring safe practice and that the task is within the scope of practice ofthe individual)
- x. Understand practice and local policies for substanceabuse and addictive behaviour, referring patients appropriately
- y. Deliver opportunistic health promotion where appropriate
Theremay be, on occasion, a requirement to carry out other tasks, this will bedependent upon factors such as workload and staffing levels

- Masters degree in Advanced Clinical Practice (or working towards/willingness to undertake).
- Experience
- Significant experience working as an autonomous advanced clinical practitioner.
- Proven experience working within a general practice or other primary/urgent care environment.
- Experience managing patients with complex and long-term conditions.
- Excellent communication and interpersonal skills.
- Advanced clinical assessment and diagnostic skills.
- Strong clinical decision-making and problem-solving skills.
- Ability to work both independently and collaboratively within a multidisciplinary team.
- Proficiency in IT and clinical record-keeping.

- Full UK driving licence and access to a vehicle.

- Experience in change management or service improvement initiatives.
- Experience with specific clinical systems (e.g., SystmOne).
