Responsibilities of this role:
  • Complete Care Act assessments and support planning for individuals preparing to leave hospital.
  • Work closely with health professionals, discharge coordinators, and community services to facilitate smooth transitions.
  • Identify risks and safeguarding concerns, implementing robust plans to protect vulnerable adults.
  • Arrange care packages, placements, and community support to promote independence and well-being.
  • Ensure compliance with the Mental Capacity Act and best interest decision-making where required.
  • Maintain accurate records and contribute to multi-disciplinary discharge planning meetings.
  • Respond promptly to urgent cases and manage competing priorities in a fast-paced environment.
Requirements of this role:
  • Must be a Qualified Social Worker, registered with Social Work England.
