Job Description for Adults and Children Safeguarding Care Coordinator Job Title Adults and Children Safeguarding Care Coordinator Line Manager Practice Manager Partner/Safeguarding Lead Contract Type Permanent Working Hours 20 hours per week Location Working across 4 surgeries Job Summary Job Description Purpose of the role The Adult and Children Safeguarding Care Coordinator plays a central role in supporting safe, effective and consistent safeguarding practice across the practice and PCN. The post holder will coordinate safeguarding processes for adults and children, ensuring safeguarding lists are accurate, actions are followed up, information is shared appropriately, and governance requirements are met.
The role supports Safeguarding Leads by maintaining oversight of safeguarding activity, coordinating meetings, audits and reporting, and ensuring compliance with local and national safeguarding guidance.
The post holder will work closely with clinical teams, safeguarding leads, social care, and partner agencies to ensure safeguarding concerns are identified, recorded, escalated and reviewed in a timely and robust manner.
This is a non-clinical but essential coordination and governance role within the multidisciplinary team.
Main Responsibilities Maintain accurate and up-to-date safeguarding registers and lists for both adults and children, ensuring regular review and follow-up for safe practice.
Coordinate safeguarding processes relating to: Children subject to Child Protection Plans Children in Need Looked After Children Children in Care Adults subject to Section 42 safeguarding enquiries or plans
Track and follow up new safeguarding referrals, including MARF, MARAC and MASH referrals, ensuring outcomes and actions are recorded and escalated appropriately.
Monitor safeguarding cases pending action, including awaiting MASH outcomes, case conference reviews or escalation decisions.
Coordinate regular safeguarding meetings, including preparing agendas, triaging cases prior to meetings, and ensuring all relevant internal and external attendees are informed and invited.
Record, disseminate and follow up actions from safeguarding meetings and reviews, ensuring accountability and timely completion.
Ensure safeguarding coding on clinical systems is accurate and aligned with local safeguarding guidance.
Support consistent, high‑quality information sharing in line with local safeguarding protocols and information governance requirements.
Standardise safeguarding reporting processes across the practice and PCN. Support safeguarding audits and assurance work as required.
Ensure safe, accurate record keeping using SystmOne, including documentation of safeguarding discussions, meetings and reviews.
Support Safeguarding Leads by ensuring dedicated time is protected for complex case discussion, reflection and escalation.
The Safeguarding Coordinator is responsible for disseminating safeguarding information and delivering training to Safeguarding Administrators when processes are updated.

You are responsible for delivering a compassionate, dignified and respectful service to patients at all times.
Duty of Candour You must adhere to the principles of openness, transparency and the statutory duty of candour in your day to day work and conduct and encourage the same behaviours within the PCN. Infection Control The post holder is accountable and responsible for the prevention of healthcare associated infections by complying with all Infection Prevention & Control policies and procedures in line with legislation (Health Act 2006; Code of Practice for the Prevention and Control of Healthcare Associated Infections). Health and Safety All employees must be aware of the responsibilities placed upon them under the Health and Safety at Work Act (1974) to ensure that the agreed safety procedures are carried out to provide a safe environment for employees and visitors. GDPR The General Data Protection Regulation requires you to ensure compliance with all relevant policies and those procedures relevant to the area of work. The PCN will always seek to process your personal data in accordance with its obligations and your rights. Confidentiality Your attention is drawn to the confidential nature of information collected and used throughout the NHS. The unauthorised use of disclosure of patient, staff or other personal information is a dismissible offence. The unauthorised disclosure of information could also result in a prosecution for an offence, or action for civil damages, under the General Data Protection Regulation. You are required to observe the strictest confidence regarding any Confidential Information relating to work of the PCN, its patients and its employees. Risk Management All staff working in, or for the PCN have a responsibility for participating in the risk management programme. All post-holders have a responsibility to assess all risks to systems, processes and environment and contribute to the clinical and corporate governance agendas as appropriate. Safeguarding Duty It is the responsibility of the post holder to be aware of and follow the legislation and guidance regarding Safeguarding Children and Adults. The post holder is responsible for ensuring they receive the appropriate level of Safeguarding training according to their role. Information Technology It is the responsibility of the post holder to have a level of IT competence relevant to their job role and will be expected to continue to keep their skills up to date as part of their Continuing Professional Development.
