A healthcare provider in Wales is seeking a Care Navigator to support their clinical team by directing patients to the right healthcare services. The ideal candidate will manage calls and appointments while maintaining patient records in a modern health centre. Attention to detail, strong communication skills, and ability to work well in a team are essential. This role offers flexible work hours within a supportive environment.

Hours: 28.5 hours a week (4 days per week)
To support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the practice’s multidisciplinary team.
The Care Navigator will act as the central point of contact, be responsible for the distribution of information, messages and enquiries for the clinical team, liaise with multidisciplinary team members and external agencies such as secondary care and community service providers. The role also includes reception duties to support the team in delivering a professional service to the entitled patient population—welcoming patients, assisting with total triage, patient registration, booking appointments, processing electronic and hard‑copy information in a timely manner and in accordance with current policies.

Well‑organised IT and efficient practice using Vision Clinical system. Located 25 minutes from Cardiff with good transportation links.
