Position Summary:
Provide home support services, including assistance with activities of daily living, delegated tasks after transfer of function training, meal planning and preparation, and light housekeeping.

Responsibilities:
- Follow Care Plans and maintain accurate client records.
- Build effective relationships with clients to promote independence.
- Understand and execute delegated tasks responsibly.
- Communicate client condition changes to supervisors and collaborate with team members.
- Prioritize client safety and seek support from Nurse Supervisors when needed.
Qualifications:
- Community Health Worker (CHW) or Health Care Aide (HCA), certified with BC Care Aide Registry.
- Healthcare experience.
- Current First Aid/CPR (Level C).
- Valid driver’s license and vehicle preferred.
- Immunization record (including TB test within past 12 months).
- Vulnerable sector check upon hire.
- Availability for days, evenings, weekends.
- Ability to work independently and in teams, with strong organizational skills.
- Excellent interpersonal, communication, and problem-solving skills.
- Basic tech skills are an asset.
- Physical ability to perform duties, including lifting up to 55 pounds, walking, climbing stairs, and standing.
