Reporting to the Manager, Health Services, the Social Worker participates in the development, implementation, and evaluation of social work programs and services designed to deliver quality, client-focused care consistent with the mission, vision and values of Fraser Health.
The Social Worker acts on referrals from staff, community based agencies, self and/or family referrals, conducts comprehensive social and psycho-social assessments and liaises with community agencies to ensure optimal continuity of care for patients and residents.
Master’s Degree in Social Work from an accredited school of Social Work. Three (3) years’ recent, clinical experience or an equivalent combination of social work education, training and experience.
Current full registration with the BC College of Social Workers. Valid Class 5 driver’s license and access to a personal vehicle for business related purposes.
Applicants with a Bachelor of Social Work (BSW) degree from an accredited School of Social Work, and a demonstrated equivalent combination of education, training and experience may be considered. If you are applying under equivalency, please clearly outline in your application how your knowledge, education, skills and experience meet Master of Social Work (MSW) equivalency.
