Overview

Sentara Health is hiring a Social Worker for Hospice Services in Halifax (South Boston), Virginia.

Shift: Days, Part Time (24 hours / week with an on call requirement on weekends)

Location: South Boston, VA

Responsibilities
  • Performs a variety of casework and provides social work services to patients, families, and significant others. Develops and monitors multidisciplinary collaboration of services to patients where appropriate. Assists adult patients and their families with personal and environmental difficulties associated with medical conditions, up to and including end-of-life concerns.
  • Manages patient-related referrals and inquiries. Assesses and evaluates patient / family emotional, social, spiritual, and economic needs. Establishes a plan of care to meet identified needs and utilizes care plans or teaching guides to implement and direct patient care.
  • Performs thorough and accurate assessment of patient / family needs. Identifies measurable outcomes with specific interventions. Makes appropriate referrals to community agencies, nursing homes, etc. for follow-up after discharge. Evaluates and investigates patient financial resources and alternate funding sources. Assists team members in understanding social and emotional factors related to health. Participates in ongoing discharge planning.
  • Completes accurate and timely documentation of care and / or services provided. Maintains thorough communication with internal and external customers as well as patients and families.
  • Works in settings such as social work in physician office settings, CSB, home health, long-term care, acute care, or managed care (experience in relevant settings).
  • Engages in the care planning process and participates in ongoing discharge planning.
Qualifications
  • Bachelor's Degree - Social Work
  • Driver's License
  • BLS (within 90 days of hire)
Experience
  • 1+ year of experience as a Social Worker
  • Previous Hospice / Home Health experience
