Job summary

A healthcare services organization in Gravenhurst seeks an RAI Coordinator RPN to support team members in achieving MDS accuracy and compliance. The ideal candidate will have a healthcare qualification, experience in clinical settings, and strong analytical skills. This role is essential in enhancing resident care through effective data management and team support.

Qualifications

  • Minimum of 3 years clinical or project experience required.
  • Successful completion of RAI MDS certification as per Ministry of Health is mandatory.
  • Must pass the prescribed Vulnerable Sector Check.

Responsibilities

  • Act as the RAI MDS 2.0 resource for team members.
  • Establish auditing processes for RAI MDS accuracy.
  • Review and analyze monthly RAI MDS for each resident.
  • Handle the computerized Data Submission process.
  • Monitor RAI outputs and communicate with management.
  • Perform other assigned duties.

Overview

Responsibilities
  • Act as the RAI MDS 2.0 resource for all participating team members in the Care Community.
  • Establish an effective process for auditing to ensure RAI MDS accuracy and compliance with coding, rapping and care plan updating.
  • Review and analyze monthly RAI MDS for each resident to ensure accuracy of assessments and meeting of resident needs.
  • Assume responsibility for the computerized Data Submission process and maintain records following each submission within time frames outlined by the Ministry of Health and Long-Term Care.
  • Monitor and analyze RAI outputs and reports and communicate with the management team and Support Services Office committees.
  • Perform other duties as assigned.
Qualifications
  • Current general certificate of registration with a College of a Regulated Health Profession or Ontario College of Social Workers with a minimum of 3 years clinical, project experience.
  • Successful annual completion of RAI MDS certification as per Ministry of Health requirements for AIS.
  • Problem solving, assessment, and analytical abilities are required.
  • All applicants must successfully pass the prescribed Vulnerable Sector Check.
Bonus Points
  • MDS education is an asset.
  • Previous experience in adult education and facilitation is considered an asset.
