Job Summary
As the Registered Practical Nurse, you will be part of an interdisciplinary team expected to ensure the safety and security of the resident environment, practice in accordance with College of Nurses of Ontario professional standards, and adhere to policies and procedures defined by LOFT Community Services. The RPN will also contribute to program development, participate in clinical supervision, and provide support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors, enhancing their dignity and personal empowerment. The RPN is responsible for developing strategies for the management of recovery and psychosocial rehabilitation, liaising with community agencies, other practitioners, and key partners to coordinate care for clients.
LOFT was chosen to be the Homecare and Community Services provider for CAMH's new Hospital to Home (H2H) program. The standard Service Period for a H2H Program is sixteen (16) weeks, with one Service Bundle assigned to the Patient and delivered over the defined timeframe, following the Patient Transitional Care Plan. The H2H program supports adults 55+ with complex mental health challenges, addiction, dementia and responsive behaviours, along with physical health needs. As part of this mobile team, staff travel across the GTA to support individuals in their homes.
The Path Home (TPH) is a 48-unit ALC transitional housing program offering supportive care to individuals living with mental health challenges. The program provides medical monitoring, wound care, medication management, primary care services, case management, life skill training, and community engagement activities.
What You Will Do
- Provides direct nursing care with medications as defined by the College of Nursing Practice including
- administration of medications through oral, subcutaneous, topical, natural body orifices, intracutaneous, tubing routes
- transcribing medical orders as given by physicians in writing or by telephone
- administration of routine medications including narcotics
- administration of routine p.r.n. medications and identifying when the order needs to be adjusted
- maintains security of medication system and storage
- orders medications as required and maintains the MAR system
- follows all policy and procedures as outlined by the Pharmacy
- Identify changes in appearance or behaviours, reporting physical and mental health concerns, unusual or exceptional occurrences and behaviour
- Participate in service planning and development of personal goals for clients to transition to their permanent discharge location
- Provide support with behaviours arising from cognitive issues, intervene in conflicts between residents, and contact appropriate emergency services in crisis situations
- Maintain all records of service and ensuring supplies and equipment are readily available
- Conduct mandatory assessments and provide timely and accurate record keeping and reports as required
- Ensure proper care is rendered in a safe and clean environment
- Liaise with physicians, pharmacists and other health care providers regarding client care
- Provide advice, coaching and support to staff regarding medical/healthcare questions and concerns
- Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles
- Maintain strict confidentiality and follow all other required policies and procedures
- Consistently interact with clients, staff, family members, community agencies and partners in a professional manner that reflects favourably on LOFT Community Services
- Liaise, collaborate and coordinate with other professionals, service providers and partners within the Program
- Participate in on call rotation regularly and as required
- Follow standard infection control practices and enhanced precautions as instructed
- Effectively participate as a member of the LOFT Team; attend staff/team meetings, client care conferences, other meetings etc. as required
- Serve as back up to Personal Support Workers, as required
- Attend and actively participate in all required training
- Complete other duties, as assigned by the Director; Support Services and/or Director
What You Bring
- Currently registered with the College of Nurses of Ontario
- Graduate of a recognized program for Registered Practical Nurse
- Medication Administration Certification or completion of an RPN education preparation program including medication administration
- Experience with vulnerable older adults and seniors with serious mental illness, cognitive, addiction and responsive behavioural challenges
- First Aid and CPR certification
- Knowledge of social services and community resources and supports for adults and seniors
- Criminal Records Check and Vulnerable Sector Screening
- An understanding of recovery and psychosocial rehabilitation principles and the therapeutic relationship
- Well-developed problem-solving abilities and interpersonal skills
- Self-directed, results oriented and capable of working independently as well as in a team environment
- Physically able to perform demanding body mechanics (i.e. lifting, transferring, Hoyer, etc.)
- Counselling, responding and assessment skills
- Good oral and written communication skills and computer literacy, including MS Word and Excel
- Ability to collaborate effectively with other members of inter-professional care teams, clients, family members and community team members
Work Environment
- Physically able to perform demanding body mechanics i.e. lifting, transferring, Hoyer, etc.
- Staff will be required to work in client’s own homes and may therefore be exposed to secondhand smoke
- Occasional evening and weekend hours may be required
- Expected to meet people outside, making exposure to the weather an expectation of the position
- A car is not a requirement of the position however driving for prolonged periods of time with frequent stops is an essential part of the job. You may need to walk on rough terrain from time-to-time, for example uneven ground, parks, or ravines while providing outreach to community members sleeping outside
