Qualifications

• Minimum 3-5 years clinical experience as a Registered Nurse.
• Recent experience in Pediatrics, Chronic Disease Management, or Cardiology.
• Valid CPR Certification required.

Responsibilities

• Provide first in-home nursing visit within 24 hours post-hospital discharge.
• Complete clinical assessments and confirm discharge plans.
• Conduct medication reconciliation and provide patient care education.
