We are looking to recruit amotivated, caring and enthusiastic Practice Nurse with excellent communication and interpersonal skills to join our friendly andsupportive nursing team, 20-28 hours a week.

You will be responsible forensuring the delivery of safe and effective nursing care to the whole practicepopulation, working closely with the wider Practice Team to deliver thepractice priorities.

Main duties of the job

Clinically, the focus of the roleis management of patients with long-term conditions, treatment room duties and general nursinginterventions for all patients. It is desirable you have knowledge of DiabetesManagement, Asthma/COPD management, Hypertension, Family Planning/contraception, cervical cytology, long term condition reviews, wound care, childhood immunisations and vaccines.

We are part of Farnham Integrated CareServices working in collaboration with local practices offering same day accessfor urgent care along with MSK Practitioners and Home Visiting Services.X-ray, ultrasound and phlebotomy services available within Farnham Centre forHealth. We are also a Training Practice. CQC rated 'good' in November2019.

Job responsibilities

Assess, plan, develop, implement and evaluate programmes to promotehealth and well-being, and prevent adverse effects on health and well-beingImplement and evaluate individual treatment plans for patients with a knownlong-term condition

Identify, and manage as appropriate, treatment plans for patients atrisk of developing a long-term condition

Prioritise health problems and intervene appropriately to assist thepatient in complex, urgent or emergency situations, including initiation ofeffective emergency care

Support patients to adopt health promotion strategies that encouragepatients to live healthily, and apply principles of self-care

Deliver opportunistic health promotion usingopportunities such as new-patient questionnaire and health checks to assess patients alcohol consumption and invite in accordingly

Assess and care for patients presenting with uncomplicated wounds

Support and advise women requesting information relating to familyplanning needs

Support and manage health needs of women presenting for cervicalcytology consultations

Undertake ECGs.

Recognise, assess and refer patients presenting with mental health needsin accordance with the National Framework (NSF) for Mental Health

Implement and participate in vaccination and immunisation programmes forboth adults and children

Advise, support and, where appropriate, administer vaccinations forpatients travelling abroad

Organise and run chronic disease management clinics populated withpatients from the Practices' clinical registers in Asthma, CHD, COPD, Diabetes,Hypertension, Mental Health and other clinical areas as advised from time totime. Clinics are to adhere to guidance issued by the Practice from time totime, comply with NICE Guidelines and will require data recording using theclinical system protocols.

Qualifications
  • Either a recognised nursing diploma or BSc Current registration with the NMC and RCN.
  • Recognised and accredited training programmes in Liquid Based Cytology and Family Planning, Warwick Diploma in Diabetic Care or other diabetic qualification Asthma/COPD recognised qualification
Experience
  • Experience of working in accordance with the NMC Code of Professional Conduct Experience with IT systems to record patient care episodes Experience of dealing with the public/patients Experience of multi-skilled approach to Chronic Disease Management
  • Experience of working in General Practice Experience of working with Emis clinical system
