Physiotherapist – Community Advantage Rehabilitation (CAR) – a division of Lifemark

Employment Type: Part Time hours

Location: Whitby - Durham Region, ON

Lifemark’s Home and Community Care division, Community Advantage Rehabilitation (CAR) is looking for Registered Physiotherapists to provide services to our Adult clients within the community. The position can be a flexible one, with working hours ranging from part-time to full time depending on your desired client caseload size.

Responsibilities
  • Provide ministry funded, community based Physiotherapy services within the client’s place of residence and / or through virtual services
  • Provide physiotherapy assessment (through interview, standardized and informal assessment techniques) in the areas of geriatrics, neurology, orthopedics, palliative care, etc.
  • Develop and implement goal-oriented treatment plans in collaboration with clients / caregivers
  • Assign patient care plan, and provide supervision and monitoring of physiotherapy assistants when appropriate
  • Effectively communicate information by providing education to client / caregivers and linking to community agencies / resources when appropriate
  • Complete and maintain current and accurate documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and College of Physiotherapists of Ontario standards

Requirements
  • Registered Physiotherapist with the College of Physiotherapists of Ontario (required)
  • Experience working within a community setting is an asset but new graduates are welcome
  • Ability to work independently and as part of an inter-professional team
  • Ability to prioritize, manage self-scheduling and meet reporting deadlines
  • Excellent oral, written and report writing skills
  • Proficiency with computers
  • Strong interpersonal and organizational skills
  • Reliable transportation is required for this position
