  • Provide physiotherapy assessment and treatment in community settings, including cardiovascular and pulmonary rehabilitation as applicable.
  • Develop and implement individualized treatment plans and education for clients and families.
  • Collaborate with interdisciplinary teams and community partners to support client goals.
  • Document progress and outcomes, and adjust plans as needed.
  • Maintain professional practice within scope and adhere to relevant standards and regulations.
Qualifications
  • Registered Physiotherapist with applicable provincial college (e.g., CPO, Ontario).
  • Experience in community/home care settings preferred.
  • Strong communication, problem-solving, and compassionate patient-centred approach.
