Location: White Earth Health Center, Ogema, MN & Nay-Tah-Waush Clinic (Mahnomen, MN)Schedule: MondayFriday, 8-hour shifts between 7:00AM4:30PM (No overtime)
SummaryThe Physical Therapist will provide comprehensive rehabilitation services in an outpatient clinic environment serving Native American patients across the lifespanfrom infants to the elderly. This role independently evaluates and treats patients with orthopedic, neurologic, post-surgical, and chronic conditions, develops treatment plans, documents in the EHR, and participates in interdisciplinary care with clinic providers and medical staff.
Primary Duties-Perform initial PT evaluations and establish individualized treatment plans-Deliver direct hands-on physical therapy treatment for acute and chronic conditions-Manage caseload of approximately 10 patients per 8-hour day-Provide education to patients and families regarding exercises and functional mobility-Collaborate with physicians and interdisciplinary providers to optimize outcomes-Update EHR documentation for all patient encounters and progress-Participate in orientation, on-site training, and mandatory IT/HIPAA security training-Maintain a safe, clean, and professional clinical environment-Adhere to clinic policies, safety standards, schedule protocols, and infection control-Submit billing and credentialing documentation as required
Required Qualifications-Graduate of an accredited Physical Therapy program-Active, unrestricted Physical Therapy license in a U.S. state or territory-Current BLS certification-Ability to complete federal background investigation and credentialing-Proficiency with computers and ability to learn EMR system within two weeks-Strong interpersonal and communication skills-Ability to treat pediatric, adult, and geriatric patients
Preferred Qualifications-Prior experience working in IHS, tribal health, or rural community settings-Previous credentialing or work at White Earth Health Center
$100 - $120 an hour
