Program Overview – Cedarvale Housing Support, Community Care

(540 Cedarvale Avenue, Toronto, ON)

The Cedarvale House Supportive housing program provides housing for seniors who have experienced homelessness and may be living with mental health and/or substance use challenges. The program provides onsite psychosocial, primary care supports and services including intensive case management; harm reduction and addiction support; therapeutic and supportive counselling; 24-hour staffing; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up support.

Personal Support Workers (PSWs)

Personal Support Workers (PSWs) play a vital role in supporting individuals who need assistance with daily living activities (ADLs) and instrumental activities of daily living (iADLs). Guided by an established care plan, PSWs provide personalized support that respects each client’s abilities and promotes independence wherever possible. In this role, you will help foster dignity, enhance quality of life, and reduce social isolation for seniors and adults who may have physical, mental, health, or cognitive disabilities. You will work with compassion and cultural sensitivity, ensuring care is inclusive and responsive to the diverse needs of our community, including those who have experienced systemic barriers or marginalization.

What You Will Do
  • Deliver client-centered, culturally competent personal care that promotes dignity and independence.
  • Assist with activities of daily living (ADLs) such as feeding, transfers, bathing, skin care, oral hygiene, toileting, and continence care.
  • Support instrumental activities of daily living (iADLs) including light housekeeping, laundry, meal preparation, and bulk shopping.
  • Provide medication assistance or reminders in line with policy.
  • Perform delegated tasks (controlled and non-controlled acts) as taught by a Regulated Health Professional and in accordance with policies.
  • Observe and document changes in clients’ physical, mental, or cognitive health and report unsafe conditions promptly.
  • Maintain accurate progress notes and communication logs in client files and management software.
  • Communicate and collaborate with caregivers and stakeholders to ensure preferred approaches to care.
  • Respond appropriately to changes in behaviour and escalate to management when needed; work with interdisciplinary teams to support clients.
  • Apply proper food handling, infection prevention, and personal protective equipment practices.
  • Monitor program supplies and report shortages to the Manager.
  • Respond appropriately to emergencies following established protocols.
  • Participate in supervision, training, and team meetings; seek clarification on policies or incidents when required.
  • Foster teamwork and uphold anti-racism and community development principles within the program.
  • Maintain ongoing education and training as directed; represent the unit on internal committees when assigned.
  • Due to WoodGreen's multi-site locations, we reserve the right to relocate the incumbent to another site, in the same position performing the same duties, as may be dictated by program needs. Reasonable notice of such site relocation will be given.
Working Conditions
  • This position includes some evening, and weekend shifts each month.
  • Work environment may involve exposure to second-hand smoke, odors, pests, and situations where clients use substances or may experience incontinence.
  • Clients may express distress through language or behavior, including swearing.
  • Ability to lift up to 40 lbs is required.
