FJ Davey Home, located in
Sault Ste. Marie, ON.
What You’ll Be Doing- Perform comprehensive, person-centered health assessments of residents on admission, annually, and as needed (including quarterly medication reviews) to develop personalized care plans in collaboration with residents, families, interdisciplinary teams, and other health professionals.
- Evaluate the effectiveness of interventions and adjust care plans as necessary in collaboration with residents or SDMs, families, and the interdisciplinary team.
- Perform appropriate screening and diagnostic investigations, interpret results, and assume responsibility for follow-up actions.
- Accurately document assessments, care plans, interventions, and evaluations of resident care.
- Promote clinical knowledge development among staff by integrating best practices, providing leadership, and involving the interdisciplinary team in quality improvement initiatives.
What You Bring- Proof of current registration with the College of Nurses as a Registered Nurse in the Extended Class.
- Experience in Gerontology or certification from the Canadian Nurses Association is an asset.
- Knowledge of primary health care issues in long-term care facilities.
