The Role

The Licensed Practical Nurse works as a member of the multidisciplinary team, collaborating on the delivery of care to meet the needs of residents. The LPN provides care to residents whose health condition and needs are well defined and predictable with little fluctuation over time. The LPN assesses, plans, implements, and evaluates care and works in consultation with HCA and RN colleagues to meet the needs of more complex patients.

Responsibilities
  • Monitor and establish standards of care and practice to provide an environment that facilitates quality resident care delivery
  • Monitor residents’ health, note changes in condition and assess the need for referral to physicians
  • Process and implement physicians’ orders
  • Collaborate with the multidisciplinary team to develop and maintain up-to-date resident Care Plans to meet therapeutic, restorative and rehabilitative goals of each resident utilizing MDS-RAI
  • Supervise nursing care
  • Complete daily audits on Point of Care (POC) documentation
  • Administer medications according to administrative and nursing policy
  • Supervise the HCAs in assisting with medications
  • Evaluate and document the effectiveness of all medications
  • Collaborate with pharmacists
  • Observe, report, and document changes in resident status and deviations from anticipated resident responses / outcomes according to policy / charting guidelines and standards
  • Notify residents’ families or next of kin of any changes to residents’ health status
