Schedule: Monday - Friday (9 am - 4:30 pm)

Hours: 35 hours/week

Wage:$29.17 - $33.49/hour (Starting at $29.17/hr)

Opportunity

The Health Pathway Navigator (HPN) will provide culturally responsive 1-1 in-person or remote well-being navigation to clients experiencing persistent health challenges, as well as language and cultural barriers that prevent them from achieving settlement outcomes, through health care system & resource navigation & needs-based referrals. The HPN will also focus on supporting refugee and immigrant families experiencing health issues such as autism, physical or mental disability, mental health disorders, PTSD, HIV, infectious diseases, deafness and chronic illnesses. Additionally, the HPN will develop accessible health services-related resources for dissemination among newcomers, while actively updating resources with new information and feedback from clients. The HPN will support the delivery of group-based, in-person "Healthy Settlement Journey" information & orientation (I&O) workshops for newcomers from equity groups on various health-related topics such as IFHP, MSP, how to find a family doctor, immunization, & other general health topics.

The Key Areas of Responsibility are:

• Provide culturally responsive, client-centered support to immigrants and refugees experiencing persistent health challenges, language barriers, or cultural barriers;
• Deliver information on health and mental health resources (e.g., DIVERSEcity Counselling Services, Dan’s Legacy), including eligibility criteria, community programs, systems navigation, and equipment access;
• Accompany clients to medical appointments as needed, offering cultural interpretation and advocacy to ensure effective communication between clients and health practitioners;
• Support clients in navigating the Canadian health care system by providing accessible, accurate information and guidance on health services;
• Develop and maintain up-to-date resource guides related to health and well-being services for dissemination to clients and community members;
• Serve as a cultural broker and advocate to address barriers and facilitate equitable access to physical and mental health care for clients;
• Work with immigrant and refugee populations to assess health needs and connect them to relevant resources and community programs based on their individual health needs and cultural values;
• Identify other appropriate health related resources and services and refer to internal programs, external services, or community organizations as appropriate, providing ongoing follow-up to ensure gaps are bridged and clients feel supported;
• Facilitate group workshops and presentations on health-related topics, creating culturally appropriate educational materials and resources tailored for reproductive health for women, seniors care, 2SLGBTQI+specific health needs & disability health care;
• Promote client health and well-being by delivering information on health behaviors, preventive care, and wellness programs available in the community;
• Collaborate with Settlement Workers and RISE Case Specialists, to receive referrals, assist clients and address the social determinants of health;
• Build the capacity of the Settlement and Case Management staff through subject matter guidance and knowledge sharing supported through timely communication of information and resources, workshops or 1:1 support;
• Develop and administer quarterly client surveys to assess satisfaction, measure the impact of navigation services, and identify areas for improvement;
• Collaborate with the UBC Refugee Health Initiative, Elizabeth Fry Health Centre, & New Canadian Clinic;
• Conducts outreach and networking in the community with health-related service providers;
• Develop and administer post-workshop surveys to assess client satisfaction & understanding of materials presented, as well as, assess new emerging needs of newcomers to adjust session topics where needed.
• Maintain accurate client records and documentation on CRM for iCARE monthly reporting
• Adheres to agency policies and procedures and standards of practice.

Qualifications

• Degree in Health Sciences or related field;
• Minimum of two (2) years recent related experience; or an equivalent combination of education, training & experience;
• Exceptional written and verbal English language communication skills;
• Written and verbal fluency in Dari & Pashto is required;
• Understanding of the issues newcomers and vulnerable minority populations face pre- migration and during settlement;
• Extensive knowledge of health systems and programs in the Lower Mainland;
• Demonstrated understanding of GBA+ and ability to incorporate it into client care;
• Demonstrated understanding of social determinants of health and how these impact racialized immigrant and refugee populations;
• Demonstrated ability for active listening, and building trust with all stakeholders;
• Demonstrated ability to advocate for clients needs and resources internally and externally;
• Demonstrated understanding of cultural and traditional health practices and beliefs of the communities being served;
• Demonstrated ability to work independently; take initiative, solve problems and work as part of a team;
• Good knowledge of standard office equipment and software applications;
• Ability to work a flexible schedule;
• Valid Class 5 drivers license and access to personal vehicle.
