Job Summary
The Clinical Pharmacist delivers individualized medication therapy, counsels patients, collaborates with healthcare providers to develop therapeutic plans, and participates in guideline development, quality improvement initiatives, education of pharmacy students and residents, and medication safety activities.
Responsibilities
- Consults with physicians, nurses and other healthcare providers to develop and discuss therapeutic plans appropriate for each patient based on chart review, assessment of clinical responses, and modification as needed.
- Individualizes patient medication therapy by reviewing charts, laboratory results, and patient demographics, meeting with patients and the care team to assess clinical response, and modifying therapeutic plans to ensure consistency with patient needs.
- Provides patient medication counseling regarding drug therapy, including reviewing reference literature, summarizing information, and demonstrating procedures and equipment.
- Assists in developing drug therapy guidelines for disease conditions, reviewing literature, compiling data, and monitoring guideline effectiveness.
- Participates in ward rounds, program meetings, and unit-level pharmacy services to enhance patient care.
- Acts as department resource person in the assigned area, providing detailed drug information on drug therapy issues.
- Contributes case studies and review articles to professional journals by identifying topics, gathering data, and writing/submitting manuscripts.
- Provides instruction and/or supervision to pharmacy students and residents through educational material, demonstration, lecture, and performance evaluation.
- Participates in quality improvement activities, performing audits, identifying potential problems, and recommending changes.
- Performs drug distribution duties, interpreting prescriptions, dispensing medications with a computerized system, and ensuring legality, professionalism, and documentation.
- Reviews prescriptions for therapeutic suitability using standardized references and laboratory data.
- Communicates with prescribers and health team for prescription clarification, documenting outcomes in the patient record.
- Ensures efficient and accurate dispensing, labeling, and completion of dispensing records.
- Participates in medication use evaluation projects, maintaining inventories, requisitioning drugs, monitoring expiry dates, and reporting errors.
- Participates in clinical drug trials by dispensing medications, completing documentation, and reviewing adverse reaction cases.
- Participates in medication safety initiatives and quality control activities per established standards.
- Participates in staff meetings, committees, and programs, identifying opportunities to improve processes, and referring to supervisor.
- Maintains records for narcotic and controlled drugs and departmental statistics in compliance with legal and departmental standards.
- Prepares reports on activities, gathers and records information into approved formats, and manages spreadsheets and file updates.
- Performs other related duties as assigned.
Qualifications
- Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
- Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience.
