Qualifications

• Minimum 2 years of experience in community health, home care, or a hospital setting.
• Eligible for or currently holding a DHA license.
• Valid driver’s license preferred due to mobile nature of role.

Responsibilities

• Provide nursing care at patients' homes including post-operative and wound care.
• Conduct assessments and develop care plans with healthcare team.
• Educate patients and families on treatment and health maintenance.
• Monitor patient progress and communicate changes to care coordinators.
