Job summary

Placements24 seeks a Senior Community Support Worker to join a remote team in South Africa. You will assess needs, develop and implement care plans, and provide direct client support from home, coordinating with families and other professionals to ensure comprehensive service delivery.

The role emphasizes person-centred care, safeguarding, and the use of digital tools. You will mentor junior staff and lead virtual activities while delivering high-quality support across SA.

Qualifications

• Qualified in Social Work, Community Development, Psychology, Nursing, or related field.
• Minimum of 5 years in a community support or social care setting.
• Experience in conducting needs assessments and developing care plans.
• Knowledge of safeguarding policies and procedures.
• Excellent communication and listening skills.
• Proficiency with Zoom, Teams, Slack and electronic records.
• Ability to work autonomously in a remote environment.
• Willingness to undergo background checks and provide references.

Responsibilities

• Conducting comprehensive needs assessments for individuals requiring support.
• Developing, implementing, and reviewing personalized care and support plans.
• Providing direct, hands-on support in line with care plans, including daily living activities.
• Facilitating social inclusion and community participation.
• Monitoring client progress and reporting changes to supervisors.
• Collaborating with families, caregivers, and other professionals for coordinated care.
• Maintaining confidential client records using digital systems.
• Guiding and mentoring junior staff on complex cases.
• Organizing and facilitating virtual support groups and activities.
• Adhering to organizational policies and ethical guidelines.
