Job summary

Placements24 is seeking a dedicated Community Support Specialist to join a fully remote team serving the Cape Town community. You will connect vulnerable individuals with essential services, assess needs, guide resources, and provide emotional support to promote independence and well-being.

Work from home across the Western Cape, coordinating with NGOs and government agencies to strengthen client support pathways and community resilience.

Qualifications

• Degree or diploma in Social Sciences, Community Development, or a related field.
• Experience in community outreach, social support, or client services.
• Familiarity with the social service landscape in Western Cape.
• Ability to use technology for remote client engagement and case management.

Responsibilities

• Conduct remote needs assessments for individuals and families facing social challenges.
• Facilitate access to community resources such as housing, employment assistance, and healthcare services.
• Provide ongoing support and follow-up to ensure clients' needs are met effectively.
• Maintain meticulous records of client interactions and progress using secure digital systems.
• Offer crisis intervention and support through remote communication channels.
• Collaborate with NGOs, government agencies, and service providers to enhance client support pathways.
