Placements24 is seeking a highly skilled Senior Case Manager to join our fully remote team in South Africa. The role focuses on individuals with complex needs, providing comprehensive case management, planning, advocacy, and service coordination to improve quality of life.

You will assess, develop and review person-centered plans, work with mental health, disability and social welfare services, and mentor junior case managers.

Qualifications

• Master's degree in Social Work, Psychology, Counseling, or related field.
• Registration with SACSSP or relevant professional body.
• Minimum 7 years of progressive case management experience with complex needs.
• Experience in assessment, crisis intervention, and care coordination.
• Knowledge of mental health, disability services, and social welfare systems.
• Excellent communication, interpersonal and advocacy skills.
• Ability to work independently in a remote setting.
