Placements24 is seeking a highly organized Remote Case Manager to support clients in Randburg and surrounding areas. You will assess needs, develop care plans, and connect clients with appropriate resources while working from a remote location.

The role emphasizes excellent communication, problem-solving, and administrative skills. You will manage a caseload autonomously and collaborate with partners to promote independence and well-being for vulnerable individuals and families.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field.
• Minimum of 2 years of experience in case management.
• Proficiency with case management software and virtual communication tools.
• Strong knowledge of community resources and social service delivery systems.
• Excellent organizational, time management and problem-solving abilities.
• Ability to work autonomously from a remote setting and manage a caseload.

Responsibilities

• Conduct thorough needs assessments for clients via virtual meetings and phone calls.
• Develop, implement, and monitor individualized case management plans.
• Coordinate and arrange services with various internal and external providers, such as healthcare, housing, and employment agencies.
• Provide advocacy and support to clients navigating complex systems and accessing resources.
• Maintain accurate, up-to-date client records and progress notes in the designated digital system.
• Regularly review case plans and client progress, making adjustments as needed to ensure optimal outcomes.
