Placements24 is seeking a proactive Social Support Specialist to provide remote assistance to individuals and communities in the East London region and beyond. You will assess needs, guide clients to social services, and develop individualized plans while maintaining confidential records.

Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology, or related field.
• Experience in social services, case management, or client support.
• Strong knowledge of social welfare systems and community resources.
• Excellent verbal and written communication skills.
• Proficiency with remote communication tools and digital record-keeping.
• Ability to work independently and manage a caseload remotely.

Responsibilities

• Conduct remote needs assessments for individuals and families seeking support.
• Provide information and guidance on accessing social services, benefits, and programs.
• Develop and monitor individualized support plans to help clients achieve their goals.
• Act as a liaison between clients and service providers, ensuring smooth communication.
