Job summary

Placements24 is seeking a compassionate Social Care Support Worker to join our fully remote team in South Africa. You will assess client needs, provide emotional and practical support, and connect individuals with resources and services to enhance wellbeing and independence.

You will work autonomously from home, using digital tools to maintain confidential records, develop support plans, and collaborate with the remote team to share insights and best practices.

Qualifications

• Relevant qualification in Social Work or related field.

Responsibilities

• Conduct initial needs assessments via phone or video calls to understand client requirements.
• Provide empathetic emotional support and guidance to clients facing various challenges.
• Assist clients in accessing community resources, healthcare, and social services.
• Develop and monitor individualized support plans, ensuring clients' goals are met.
• Maintain accurate and confidential client records using designated software.
• Collaborate with the remote support team to share insights and best practices.
