Placements24 is seeking a skilled Social Support Facilitator on a contract basis to assist clients across the East London region. This fully remote role focuses on connecting people with social services, counseling, and community programs, and requires developing personalized support plans.

You will assess needs, arrange services, maintain confidential records, and work with partners to ensure coordinated support. The position offers meaningful impact and flexible remote work.

Qualifications

• Relevant qualification in social work, counseling, social services, or related field.
• Experience in case management or client support.
• Knowledge of social welfare systems and community resources.
• Excellent assessment, active listening and empathetic communication skills.
• Ability to work autonomously in a remote setting and manage caseloads.
• Proficiency in using digital tools for communication and records.

Responsibilities
