Job summary

Placements24 is seeking a proactive Social Support Facilitator for a remote contract. You will connect clients with housing, welfare and employment resources, assess needs, and support timely access to services in and around the Nelspruit region.

The role requires a qualification in Social Work or related field, strong communication and empathy, and the ability to manage a caseload remotely. Remote, flexible work setup and remote collaboration with local providers are part of the job.

Qualifications

• Qualification in Social Work, Community Development or related field required.
• Experience in social work, community support or case management desired.
• Strong understanding of welfare systems and available resources.
• Excellent communication, active listening and interpersonal skills.
• Proficient with computers, internet and remote communication tools.
• Able to manage a caseload remotely with empathy and organisation.
• Familiarity with Mpumalanga community needs is a plus.

Responsibilities

• Provide remote support and guidance to individuals and families seeking social services.
• Assess client needs and identify appropriate resources and programs.
• Assist clients in completing applications for benefits, housing, employment, and other services.
• Maintain regular contact with clients via phone, email, and video conferencing.
• Educate clients about their rights and available options.
• Liaise with service providers to facilitate access to resources.
• Maintain accurate and confidential client records and document interactions.
• Advocate for clients' needs within social service systems.
• Stay informed about available programs and eligibility criteria.
• Contribute to service delivery improvements through feedback and case reporting.
