Placements24 is seeking a compassionate Social Support Facilitator to work remotely, serving Port Elizabeth and the Eastern Cape. You will conduct virtual needs assessments, link clients to services, and provide empathetic support to help build action plans.

Collaboration with agencies and delivery of online workshops are key parts of the role. Ideal candidates have a Diploma or Bachelor's in a related field and at least 3 years of experience in social work or community support.

Formación

• Diploma or Bachelor's degree in Social Sciences, Community Development, Psychology, or a related field.
• Minimum of 3 years of experience in a social work, counseling, or community support role.
• Proven ability to connect with and support individuals from diverse backgrounds.
• Excellent verbal and written communication skills with strong active listening abilities.
• Proficiency in using virtual platforms (Zoom/Teams) and standard office software.

Responsabilidades

• Conduct virtual needs assessments to identify client challenges and goals.
• Provide information and referrals to social services, healthcare providers, and community resources.
• Offer empathetic listening and emotional support to clients.
• Assist clients in developing personal action plans to address their needs.
• Facilitate access to programs and services, helping clients with applications and documentation.
• Follow up with clients to ensure services meet their needs and provide ongoing support.
• Maintain confidential client records and document interactions accurately.
• Collaborate with other service providers and agencies to coordinate care.
• Identify emerging community needs and advocate for resources.
• Deliver virtual workshops or information sessions on relevant topics.
