Job summary

Placements24 is seeking a Remote Case Manager for social services serving clients across South Africa, with focus on Mahikeng, North West. This is a fully remote, full-time role requiring assessing client needs, developing care plans, coordinating services and monitoring outcomes from a home office.

You will liaise with clients, families, and service providers, advocate for client rights, maintain confidential records, and participate in virtual team meetings.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology or related field is required.
• Minimum of 2 years of experience in case management or social services.
• Demonstrated understanding of social service systems and community resources.
• Excellent virtual communication, interpersonal, and client management skills.
• Proficiency with case management software and virtual collaboration tools (e.g., Zoom, MS Teams).

Responsibilities

• Conduct comprehensive assessments of client needs via virtual platforms (video calls, phone).
• Develop, implement, and regularly review individualized case plans with clients.
• Coordinate and facilitate access to community services (healthcare, housing, employment, education).
• Monitor client progress and adjust care plans as needed.
• Serve as liaison between clients, families and external providers with clear communication.
• Maintain confidential electronic case notes in line with policies.
